Job Description
We are a Targeted Case Management Agency serving individuals with intellectual and developmental disabilities in Cole County. This is a very rewarding position linking individuals with the services and supports they need to be inclusive in the community in all aspects of their lives. Time management, excellent verbal and written communication skills are essential to be successful in this position. We will help support you to be successful in this position with extensive training necessary to help meet the needs of others.
JOB DUTIES

1. Manages a caseload of 35 to 40 individuals normally involving a number of disability areas.
2. Holds meetings with clients, their families and other responsible individuals; assists in completing the application for services; collects basic data and obtains appropriate additional information from other agencies; participates on the inter-disciplinary team to review each individual's progress and goals.
3. Prepares daily case progress notes, and other reports concerning the client’s case.
4. Develops, in cooperation with other staff, an Individualized Support Plan for each client.
5. Attends inter-disciplinary treatment team meetings to discuss client cases in relation to eligibility, support plans, progress and possible changes in the service program.
6. Prepares budgets for clients to access services.
7. Monitors services to ensure that the terms of the authorization are being fulfilled by the vendor, to check on quality of services and to review client progress.
8. Requests modifications for a non-productive program or enrolls the client in an appropriate program.
9. Provides progress reports and counseling to clients, their families and other responsible individuals.
10. Attends monthly staff meetings.
11. Visits service agencies and confers with team members in order to become acquainted and informed concerning community resources for people who have developmental disabilities.
12. Performs other related duties as assigned.
13. Remote work after 6 months and meeting billing requirements is possible.
EXPERIENCE AND EDUCATION QUALIFICATIONS
Must have a bachelor’s degree from an accredited college or university.
