Role: Advance Practice Registered Nurse
Schedule: Days; 40 hrs/week with on-call coverage
Job Responsibilities:
- The Advanced Practice Registered works with a team of professional social workers, clinicians, case managers and others to ensure integrated care for adults experiencing serious mental illness.
- Establish therapeutic relationships to facilitate client progress with treatment goals.
- Have the demonstrated ability to balance independent clinical decision making with appropriate consultation from providers.
- Complete clinical assessment evaluations and re-evaluations
- Develop individualized plan for treatment based on clinical assessment.
- Provide medication evaluation and management, medical consultation with ongoing monitoring of effects of medication use in coordination with primary care providers as necessary.
- Ensure a coordinated and responsive treatment planning and evaluation process for assigned clients.
- Communicate changes in client treatment planning, medical needs and service needs to all staff involved in the provision of core services to clients.
- Ensure compliance with all policies, procedures and protocols related to the provision of psychiatric services to clients.
Qualifications and Competencies:
- Master's Degree in Nursing along with the appropriate Nurse Practitioner certification required.
- Unrestricted, current, and complete State of Vermont APRN license required (or ability to attain upon employment).
- Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
- Successful completion of a criminal history background check.
- Experience working with adults with severe and persistent mental illness, substance use disorders, developmental disabilities, and co-occurring developmental disabilities and/or medical conditions required.
- Ability to accurately assess client's presenting concerns.
- Demonstrated current knowledge of psychiatric medications.
- Solid computer skills with familiarity with operating an Electronic Medical Record required.
