San Luis Valley Health is seeking a full-time Behavioral Health Consultant to provide behavioral health support in the San Luis Valley Health's rural health clinics. The Consultant will be responsible for working in an integrated behavioral health/primary care setting, within a “hybrid model” - administering screenings, brief interventions, coordination of care, and ongoing therapy services to a panel of patients.

What You’ll Need:

  • Master’s degree in field of mental health (such as counseling or social work), required
  • Licensed in the State of Colorado, or in process, preferred
  • Trained in crisis and emergency intervention, preferred
  • BLS Certification required
  • Must be credentialed or eligible to participate in State and Federal healthcare programs and private insurances
  • Knowledge of acute and chronic care protocols and in the delivery of behavioral health care
  • Knowledge of how to assess patient health
  • Knowledge of health promotion principles, techniques, and patient learning measurements
  • Strong communication skills – clear and professional communication with patients, providers, colleagues, and administration
  • Comfortable with being a self-starter and taking initiative to respond to the needs of the team
  • Comfortable working in a fast-paced medical setting
  • Skilled at multi-tasking, organization, and attention to detail
  • Competent in using basic technology

What You’ll Do

  • Travel to the three Rural Health Clinics of San Luis Valley Health
  • Work as part of a multidisciplinary care team, assigned to a group of providers/panel of patients
  • Conduct proactive chart scrubbing to identify patients in need
  • Attend daily huddles with clinic teams
  • Perform proactive patient screening to identify patient needs
  • Provide brief interventions and skills training in exam rooms
  • Facilitate warm handoffs with primary care providers, and facilitate coordination of care
  • Conduct psychosocial assessments to support ongoing needs, discharge planning, and treatment planning
  • Provide brief, continued therapy for patients, as identified through screenings and/or provider referral to reach treatment goals
  • Refer and connect patients to community resources
  • Provide basic crisis intervention skills to identified patients, and coordinate further emergency response
  • Facilitate communication between patients and providers to achieve self-management goals
  • Triage patient phone calls related to behavioral health needs, as appropriate
