GENERAL DESCRIPTION

 

Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc. the Individual Service Coordinator assesses individual needs and assures the provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring and individual advocacy.

 

JOB RESPONSIBILITIES

 

  1. Prairieland Service Coordination Inc. provides services in twenty three counties and at any given time an ISC may be requested to work in a county typically outside their caseload's geographic area.

(i.e.:  staff  illness, staff  resignation/termination)

 

  1. Schedules and conducts an intake/ PUNS interview in response to referrals, given to the ISC by their supervisor, gathering initial information as required under the PAS Manual. Conducts annual reviews as needed or required.

 

  1. When eligible, completes the PAS process, following the designated procedures, utilizing assistance from their supervisor, when appropriate, to help determine the appropriateness and current availability of service(s).

 

  1. After the initial assessment, provide linkage to other community services, when possible, for individuals found to be ineligible for PAS/Individual Service Coordination services.

 

  1. Advocates on behalf of the individual and/or family to assist the individual in obtaining services to which he/she is eligible and has chosen. Ensures the direct involvement and participation of the individual and their family in this process.

 

  1. Reviews individual's medical, residential, vocational, educational, legal and financial needs and individual choices through ISC Visiting Notes, as required.

 

  1. Schedule and complete Discovery and Person Centered Plan process annually, or as required.

 

  1. Accurately and thoroughly documents various services delivered and maintains individual files as specified by program and agency guidelines.

 

  1. Functions in a problem solving capacity with provider agencies regarding service coordination, following the ISC Conflict Resolution guideline.

 

  1. See all waiver individuals on an as required basis and complete program requirements set forth in the waiver manual.

 

  1. Coordinates specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies as appropriate. (If Bogard clients on caseload)

 

  1. Monitors services provided to Bogard class members on a monthly basis and advocates for any unmet needs. (If Bogard clients on caseload)

 

  1. Submits all required documents to Program Manager for payroll preparation.

 

  1. Participates in scheduled all staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.

 

  1. Alerts the Program Assistant of PSCI to problematic issues, trends or service gaps.

 

  1. Perform other related work as required or requested by the Executive Director and/or Program Manager.

 JOB QUALIFICATIONS

 

 

  1. A bachelor's degree in human services required and QIDP eligibility as established by Department of Human Services.

 

  1. Has the ability to organize, plan and follow a schedule on a daily, weekly, monthly and annual basis.

 

  1. Demonstrates a working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.

 

  1. Demonstrates ability to work and interact effectively with professionals, paraprofessionals, social service organizations and clients including their families.

 

  1. Must have the ability to work well in teams of co-workers and to work at their home office site using self-discipline and self motivation skills with the ability to work without direct supervision.

 

  1. Must have the flexibility to work hours outside of the normal workweek.

 

  1. Demonstrates the ability to work cooperatively with others toward a mutual goal within the objectives of programs set forth by Prairieland Service Coordination, Inc.

 

  1. Must be willing to follow directions, accept constructive criticism and accept responsibility for their own actions.

 

  1. Follows the Employee Code of Ethics Guideline by presenting oneself in a professional manner through diplomatic oral and written communication.

 

  1. Must have reliable transportation to and from work or be able to access transportation to work and to places where individuals live/work that may be assigned to your caseload; maintain a valid Illinois Driver's License and personal auto insurance and maintain a good motor vehicle record.

 

PHYSICAL EXPECTATIONS

 

  1. Must be able to read, write and communicate effectively.

 

  1. Must be able to ambulate, sit and bend.

 

  1. Must be able to carry files.

 

  1. Must be able to lift up to 5 lbs. or pull a carrier.
