Job Summary:

The Emergency Room (ER) Navigator will play a crucial role in the implementation and success of the Transition of Care (TOC) program. This evidence-based program aims to improve the quality of care and outcomes for high-risk adults with multiple chronic conditions, including older adults. An additional key program objective is avoidance of hospital readmissions for primary and complicating conditions. The ER Navigator will collaborate with external partners, physicians, support staff, and administrators, to ensure smooth transitions of care, effective communication, and enhanced patient experiences. The ER Navigator will assist patients in navigating the healthcare system post hospital discharge, provide education and support to patients and their families, and coordinate with interdisciplinary teams to initiate enrollment and optimize care management.

Responsibilities:

· Serve as a primary point of contact and liaison between patients, emergency room staff, and the TCM program providers and support staff.

· Identify adult patients in the emergency room who would benefit from the TCM program and facilitate their enrollment and/or initial appointment with TCM provider.

· Possess excellent communication skills to relate with diverse populations.

· Uses problem-solving and critical thinking skills to respond to patients’ concerns.

· Collaborate with physicians, nurses, care coordinators, social workers, and other healthcare professionals to coordinate and deliver TCM services.

· Facilitate effective communication and information sharing among healthcare providers and support staff, ensuring continuity and seamless transitions.

· Educate patients and their families about their available support services.

· Document patient interactions and outcomes accurately and timely.

· Thorough understanding and adherence to HIPAA regulations and patient privacy laws.

· Adherence to hospital policies and procedures, ensuring compliance with all relevant guidelines, protocols, and regulations.

· Adheres to all company policies and procedures.

Requirements:

Qualifications and Certifications:

· High school diploma required – Medical Assistant (MA) certificate preferred

· Basic Life Support/CPR required

· Excellent interpersonal communication, problem-solving, and conflict resolution skills

· Ability to effectively apply critical thinking skills

· Computer skills in word processing, database management, and spreadsheet desirable

· Demonstrated professionalism and good interpersonal skills

· Bilingual preferred
