Responsibilities:
As a Home Health Physical Therapist (PT), your role includes:
- Assisting physicians in evaluating patient function, equipment needs, and developing comprehensive treatment plans.
- Conducting initial and ongoing assessments at appropriate time points.
- Participating in interdisciplinary patient assessments.
- Providing ordered physical therapy services indicated in the plan of care.
