Position Summary:

The Clinical Educator and/or Preceptor guides the training, education, and evaluation of the clinical and patient care staff. They also work with Agency management to establish professional education goals and metrics. This position will collaborate with medical practitioners to incorporate nursing processes into the plan of care for specialized patients. They will perform direct specialty nursing services to patients as required, ensuring adherence to Joint Commission standards and all related policies, procedures, and guidelines.

Essential Job Functions/Responsibilities:

  1. Provides educational leadership to patients, clinicians, and patient care staff to enhance specialized patient care within the Agency.
  2. Assists patients and caregivers with educational needs, problem resolution, and health management across the continuum of care.
  3. Collaborates with medical providers, patient care staff, and unit management in the planning, implementation, and delivery of educational curricula.
  4. Provides direct specialized nursing services to an assigned group of patients, providing specialized patient care within nursing protocols.
  5. Develops patient care plans, incorporating evidence-based research and national standards; consults with and serves as a resource to ensure quality patient care.
  6. Ensuring ongoing oversight of the agency’s quality management system.
  7. Evaluating services and programs.
  8. Initiates skills development programs within the parameters of established care models; monitors trends and implements educational strategies to ensure compliance with quality standards and parameters.
  9. Educates technical and patient care staff in the use of new equipment, supplies, and instruments; coordinates in-service training and workshops for appropriate staff.
  10. Ensures proper care in the use and maintenance of equipment and supplies; promotes continuous improvement of workplace safety and environmental practices.
  11. Supports nursing care standards through adherence to existing policies and procedures.
  12. Regularly attends unit conferences and makes recommendations for improvement in patient care via the patient care conference and unit conference.
  13. Assumes responsibility for professional growth by current reading and attendance at educational programs and unit conferences.
  14. Monitors staff education and professional outreach program offerings. Recommends direction and program focus.

Position Qualifications:

1.Associate's degree in Nursing; or master’s degree in health care or related field preferred.

2.Registered nurse with current licensure to practice professional nursing in the state in which you are applying. Demonstrated ability to supervise and direct professional administrative personnel

3.At least three to five years of experience in home health care preferably in home health care management and at least one year of supervisory experience in home health care or health related programs.

4.Knowledge of current Joint Commission policy as well as state and federal legislature relating to Home Health.

5.Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
