Position Description: | The Physical Therapist is responsible for cultivating an excellent customer experience. This position is responsible for connecting with our patients through compassionate and frequent communication. They are expected to communicate frequently with the front office and administrative staff to understand and properly communicate ways to maximize the patient care experience (including proper scheduling and avoiding lapses of care). They are expected to meet or exceed the states mandate for CEUs and to actively participate in our internal education, training and case communication with Elevates peers. They are expected to go out of their way to make sure the patient has the best experience in rehabilitation and exhausts all resources and skills to make sure each patient reaches their goals.

  • Reports to President Director Responsibilities:
  • To maintain high customer satisfaction scores that exceed industry standards.
  • Maintain and foster the Elevate culture focused on teamwork, camaraderie, and learning in an enjoyable work environment.
  • Active participant in Elevate educational and mentorship programs for staff PTs.
  • Assist in Elevate Marketing initiative (Development and implementation).
  • Meet or exceed budgeted numbers and providing scheduling adjustments when necessary to offer enough available appointments.
  • Communicate internally with the Team to provide the best experience for the patient.
  • Timely, obtain, communicate, and assist patients in filling out all insurance requirements and necessary paperwork to provide the best care and prevention of a lapse in care.
  • Performs Initial Evaluation, follow up visits and re-evaluations.
  • Discharges Patients when appropriate.
  • Provides all necessary information and documents for insurance companies.
  • Provides clear communication and education of their diagnosis and prognosis. Provides clear communication to the patient of what is expected of them in order to reach their goals. This includes communication of appropriate frequency and duration of care, appropriate self-management techniques, home exercise program, and any other lifestyle modifications necessary.
  • Stays up to date with all State expected continuing education requirements
  • Provides accurate and compliant documentation and completed in the companys initiative of 24 hours.
