Responsibilities:

As a PRN Home Health Physical Therapist (PT), your role includes:

• Assisting physicians in evaluating patient function, equipment needs, and developing comprehensive treatment plans.
• Conducting initial and ongoing assessments at appropriate time points.
• Participating in interdisciplinary patient assessments.
• Providing ordered physical therapy services indicated in the plan of care.
