Hours of Job
- PT - 8 a.m. - 4:30 p.m. 48 hours per pay period.
- On Call Coverage for After Hours
- $3,000 Sign on Bonus if desired with 2-year work agreement.
- Assumes a lead role in caring for home health and hospice patients.
- Completes nursing assessments of patient and documents appropriately in patient record.
- Collaborates with physicians and other health team members in developing a Plan of Care.
- Develops an individualized Plan of Care based upon the assessment of the patient, his caregivers and his environment.
- Implements and documents clinical and technical Aspects of Care.
- Evaluates patient's response to Plan of Care -- updates Plan of Care.
- Assumes a lead role in patient/family teaching evidenced in the Patient’s record.
- Ability to work on call hours.
- Current valid driver’s license.
- Must be able to provide proof of automobile insurance.
- Graduate of an accredited school of nursing.
- License to practice professional nursing in the State of Ohio.
- Previous nursing experience required. May include acute care, long term care, home health or hospice.
- Absence of criminal background as per Ohio law.
