Hours of Job
  • PT - 8 a.m. - 4:30 p.m. 48 hours per pay period.
  • On Call Coverage for After Hours
  • $3,000 Sign on Bonus if desired with 2-year work agreement.
Duties and Key Responsibilities
  • Assumes a lead role in caring for home health and hospice patients.
  • Completes nursing assessments of patient and documents appropriately in patient record.
  • Collaborates with physicians and other health team members in developing a Plan of Care.
  • Develops an individualized Plan of Care based upon the assessment of the patient, his caregivers and his environment.
  • Implements and documents clinical and technical Aspects of Care.
  • Evaluates patient's response to Plan of Care -- updates Plan of Care.
  • Assumes a lead role in patient/family teaching evidenced in the Patient’s record.
Requirements
  • Ability to work on call hours.
  • Current valid driver’s license.
  • Must be able to provide proof of automobile insurance.
Education/Certifications
  • Graduate of an accredited school of nursing.
  • License to practice professional nursing in the State of Ohio.
Experience
  • Previous nursing experience required. May include acute care, long term care, home health or hospice.
  • Absence of criminal background as per Ohio law.
