A reputable hospital near Elkins, WV has a full-time, permanent opportunity seeking an experienced Home Health Physical Therapist to join their outstanding team environment!
Pay: $35.24 to $48.00 /hourly + Relocation assistance
Home Health Physical Therapist Opportunity:
- Monday-Friday
- Day shift/ varied hours
- No weekends, no holidays.
- Setting: Home Health
- Will be needing to travel to see patients.
- Paid Mileage reimbursement program
- Home Health Physical Therapist will see about 2-6 patients a day

Home Health Physical Therapist Job Description:
- Provide physical therapy treatment and patient care according to hospital, state and federal regulations and professional guidelines.
- For Home Health, develops Physical Therapy Plan of Care including treatments, procedures, modalities, problems, and goals.
- A visit to the patients residence at least once a month, either when the assistant is present to observe and assist or when the assistant is absent, to assess relationships and determine that the plan of cares is being followed, and the goals are being met. Documentation of supervisory visits will be maintained in the patients clinical record.
Home Health Physical Therapist Qualifications:
- Current PT Licensure in good standing
- BLS certification
- 1+ year of home health experience required.
