Licensed Physical Therapist for Outpatient Department



Traveler must have permanent address outside of radius rule of 75 miles. Please note time off requests must be disclosed with initial submission to be considered for approval. Time off requested after the acceptance date will not be approved.



13 weeks assignment, at least one years’ experience current required in outpatient physical therapy. Must be a team player, flexible and accustomed to fast paced environment. Must have successfully completed at least one 13-week travel assignment.



CERTIFICATIONS: Must have a PT License. - BLS/BCLS certifications required. BLS must be completed through American Heart Association, American Health and Safety, or American Red Cross.



SHIFTS: Starts between 0; Hours Flexible (5 x 8s; 4 x 10s, etc.) Shifts may fluctuate to fulfill department staffing needs. No Weekends, Holidays or Call. Travelers are first to float within facility and to alternate locations within 20 miles of permanent assigned location. Traveling pairs are not guaranteed same schedules or same shifts. If traveler is required to float (unlikely) they will float to support local OP Rehab facilities.



ORIENTATION: 1st week of the contract, not to exceed 40 total hours, and includes EPIC training. Scrubs required and not provided by facility
