POSITION OVERVIEW

Works collaboratively with providers, staff and other health care professionals to provide a medical home and care coordination across the health care continuum for all patients within the office setting. Is an integral member of the health care team who strives to promote patient wellness, improve care outcomes, efficient utilization of health services lowering health care costs and maintain high quality standards of care among a patient population with complex health needs. Also responsible for coordinating a wide range of self management support and disease management activities for the clinics patient population. Care Manager will be cross trained with the Annual Wellness employee.

EDUCATION

Graduate of an Accredited Nursing Program

EXPERIENCE

Minimum 2 years of nursing experience.

LICENSE/CERTIFICATIONS

MT Nursing licensure; Basic Life Support; Health Coach Certification is desirable.

KNOWLEDGE/SKILLS/ABILITIES

  • Medical Terminology
  • Basic Nursing Principles and Procedures
  • Care Management experience is desirable.
  • Evidence of essential leadership, communication, education and counseling.
  • Highly organized with ability to keep accurate notes and records.
  • Basic computer and keyboarding with Microsoft word and excel preferred.
