Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will work under the supervision of a registered nurse or physician to deliver high-quality and personalized patient care. The primary responsibilities include providing direct nursing care to patients, administering medications, monitoring patient conditions, and ensuring the overall well-being of patients. The ideal candidate will possess excellent clinical skills, strong attention to detail, and exceptional communication abilities.

Responsibilities:
1. Provide comprehensive nursing care to patients, including physical assessments, monitoring vital signs, and administering medications as per physician's orders.
2. Collaborate with the interdisciplinary healthcare team to develop and implement individualized patient care plans.
3. Ensure accurate and timely documentation of patient information, treatment plans, and progress notes.
4. Monitor and assess patient conditions, promptly identifying any changes or deterioration in order to initiate appropriate interventions.
5. Educate patients and their families on health-related topics, including disease management, medication administration, and lifestyle modifications.
6. Assist physicians and registered nurses during various medical procedures, including wound care, treatments, and examinations.
7. Respond to emergencies and provide immediate interventions to stabilize patients.
8. Maintain a clean and safe environment for patients, adhering to infection control guidelines and promoting a culture of patient safety.
9. Administer intravenous fluids and medications as per physician orders, maintaining proper infusion techniques and monitoring for any adverse reactions.
10. Collaborate with other healthcare professionals and community resources to ensure continuity of care and seamless transitions for patients.

Requirements:
1. Valid and active LPN license in the state of employment.
2. Graduate from an accredited practical nursing program.
3. Proven experience working as an LPN in a healthcare setting.
4. Knowledge of nursing principles, protocols, and procedures.
5. Ability to multitask, prioritize, and work efficiently in a fast-paced environment.
6. Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare team members.
7. Excellent problem-solving and critical-thinking abilities.
8. Proficiency in using electronic medical records (EMR) and other healthcare technology.
9. Physical stamina and the ability to lift, position, and transfer patients as needed.
10. Flexibility to work various shifts, including weekends and holidays.
