Job Description

This is a full time position located at the 3 Mile office of MLIH in Logan, WV. Regular work-related travel required, mostly in Logan and Mingo Counties in West Virginia. Must be able to work a flexible schedule.

POSITION PURPOSE: 

Must be knowledgeable about the evidence based ACT Model for adult behavioral health populations with willingness to promote and respect the philosophy of that model.  Must be able to work collaboratively as part of transdisciplinary team meeting the integrated physical and behavioral health and wellness needs of the persons served.  Must have the ability to provide culturally competent care to diverse populations.  Must demonstrate a commitment to the provision of strength based consumer-centered care and be supportive of the consumer’s own goals and motivation toward positive outcomes.  Must be organized, able to manage competing priorities, utilize strong problem solving and conflict resolution skills, and possess the ability to work effectively independent and with a team setting. 

ESSENTIAL JOB FUNCTIONS: 

  1. Serves as ACT Team Leader and provides clinical, organizational, and administrative leadership to the ACT Program. 

  1. Lead team meetings and provide on call schedule for ACT team members.  

  1. Provide direct services to individuals, families, and groups.  

  1. To work collaboratively with the ACT physician and other providers in meeting the health and behavioral health needs of the ACT consumer. 

  1. Provides crisis intervention services in office and in the community as needed.  Provides emergency on call phone support to ACT consumers and team members as needed. 

  1. Participates in daily organizational meetings. 

  1. Participates in the development of policies and procedures related to health, medications, and psychiatric care. 

  1. Assist with referrals, treatment planning, nursing duties, and prior authorizations.   

  1. A part of crisis support through telephone and VTC modalities.  

  1. Provides in-home, community, and transitional care supports/interventions to consumers involved in project as needed. 

  1. Maintain accurate and timely information in the electronic health record (EHR) and support the mission of meaningful use measures. 

  1. To serve as a consultant to team on health and safety concerns. 

  1. Performs other related duties and responsibilities as required. 

  1. Non-traditional working hours 40 hours per week.   



MINIMUM QUALIFICATIONS: 

A professional license in the state of West Virginia. Experience in Behavioral Health services with some experience in a supervisory capacity.
