Team Transformational Solutions (TTS) is a rehabilitation company that is in 25 Mission Point facilities. We employ Physical Therapist, Physical Therapy Assistants, Occupational Therapist, Certified Occupational Therapy Assistants, and Speech Language Pathologist. Position Summary:


The Physical Therapist provides a broad range of physical therapy services including assessment, treatment planning,

and therapeutic interventions in an interdisciplinary environment consistent with the position qualifications,

professional practices, and ethical standards.

Demonstrates accountability for and contributes to program development, quality improvement, problem solving and

productivity enhancement in a flexible interdisciplinary environment.

Essential Duties/Functions

• Conducts initial assessments to determine medical history, symptoms and needs.

• Contributes to interdisciplinary care plans.

• Provides appropriate therapeutic interventions.

• Evaluates patient response to treatment and adapts treatment plan as necessary.

• Documents the course of patient care including progress made and continuing need for treatment.

• Provides patient and family education on tasks and resources that will assist with continued functional

independence.

• Communicates with patients, caregivers, and their families regarding patients’ progress.

• Contributes to the delivery of cost-effective care.

• Reports and complaints/grievances made by the residents to the Therapy Program Director. Reports incidents

of suspected incidents of resident abuse to the Therapy Program Director and/or the Executive Director

immediately.

• Additional job duties as assigned.
