We are looking for a motivated, organized, and talented Senior Liaison to join our team, focusing on leading and promoting the growth and development of Community Wellness's Hospital to Home Integrated Care Program within an assigned region/territory. Job Purpose:
As a vital link between an organization and the region it serves, you will play a crucial role in building strong relationships, promoting the organization's mission, and facilitating effective communication.
Responsibilities:
- Achieves measurable, scalable volume through strategic regional partnership development and account management.
- Acts as a bridge between the organization and regional partners, fostering positive relationships, and maintaining a strong presence within the region/territory.
- Advocates for partner needs and concerns within the organization, ensuring that partner voices are heard and considered in decision-making processes.
- Facilitates effective communication between regional partner members and the organization, conveying information, updates, and resources in a clear and accessible manner.
- Collaborates with regional stakeholders to identify their needs, challenges, and aspirations, and communicate these insights to the organization to inform program development and improvements.
- Provides information and referrals to partner members seeking assistance or resources, connecting them with appropriate services or programs both within and outside the organization.
- Plans and organizes events, workshops, and meetings that promotes region-wide engagement, education, and awareness of available services.
- Mediates and resolves conflicts or issues that may arise between regional stakeholders and the organization, working to find mutually beneficial solutions.
- Gathers and maintains data on regional demographics, concerns, and feedback to support evidence-based decision-making.
- Collaborates with organizations, government agencies, and stakeholders within the assigned region to identify opportunities for partnership and collaboration on community initiatives.
- Performs other duties as assigned.
Here are our MUST HAVES:
- Bachelor's degree in Business Development, Healthcare, Public Relations, Nursing, or related field from a four year college or university and FIVE or more years related experience OR the equivalent combination of education and experience.
- At least FIVE or more years of previous experience working in a regional hospital/health system sales and/or liaison capacity
- Sensitivity to cultural diversity and an understanding of how cultural factors may impact community dynamics and interactions.
- Strong networking abilities to connect with regional leaders, organizations, and resources.
- Familiarity with the region, its demographics, and the social and economic challenges it faces.
- A deep commitment to regional development and a passion for making a positive impact on the lives of community members.
- Understands the purpose, operation, and utilization of selected virtual monitoring devices
- Excellent communication skills, both verbal and written
- Possesses a high level of commitment to meeting deadlines and benchmarks
- A planner with capable organization and prioritization skills
- Able to work well independently and within a team setting
- Strong interpersonal and relationship building skills
- Easily facilitates the exchange of information
- High attention to detail and accuracy
- Fast learner with the desire for advanced training
Preferred Experience and Skills:
- Bilingual (in desired respective language) in geographic areas where there are significant populations of non English speaking individuals

Compensation: $100,000.00 per year
