Job Description

Purpose:

In compliance with all state statutes and regulating bodies, in accordance with FHLC Policy and Procedure and within the scope of training and skill level, assess, diagnose, counsel, and provide psychotherapeutic and Substance Abuse treatment, rehabilitation, and care coordination for children, adolescents, and adults (individual, family, group); provides educational services to Providers and staff, community agencies, and the public; and performs related duties as assigned.

Essential Duties and Responsibilities:

  • Therapeutic intervention for Individuals, couples, and/ or family consultation with patients, (In-person and via Telehealth) including:

o Functional and strength-based assessment and diagnosis;

o Psychoeducation for patients and their support systems;

o Medication adherence counseling and disease self-management counseling;

o Motivational Interviewing to develop behavioral strategies aimed at symptom reduction

o Cognitive behavioral intervention aimed at modifying negative thinking and promoting self-efficacy;

o Self-Care Plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation; and

o Substance use disorder evaluation, identification of maladaptive coping strategies, and development of harm reduction strategies.

  • Interviews potential patients to assess needs for mental health services or referral elsewhere; counsels or provides psychotherapeutic and rehabilitation services for individuals, couples, or members of a family to assist them in achieving productive social adjustments, reducing the impact of disabling disorders, and facilitating their achievement of living, vocational, recreational, and interpersonal goals.
  • Documents clinical activity presenting diagnostic and interview findings, clinical evaluations, and recommendations; as a member of an interdisciplinary team, reviews diagnosis and treatment plans of clinic staff; and may act as a case manager by coordinating services for clients to assure continuity of care and to assure that clients receive appropriate types of service.
  • Confers with and advises patients’ relatives to secure their understanding of and cooperation in treatment and rehabilitation programs.
  • Cooperates, advises and works with other FHLC staff and outside agencies/practitioners regarding mutual patients.
  • Educates patients and their families regarding their diagnosis, prognosis, treatment, and rehabilitation process.
  • Manages the care of patients by assessing and referring them to public and private providers and monitoring their progress.
  • Provides crisis counseling for individuals experiencing acute episodes; evaluates patients and arranges for voluntary or involuntary admission to facilities for observation and treatment.
  • Assists patients to obtain stable housing, food security, physical health care, benefits, transportation, and other enabling services to enhance social determinants of health and improve overall health outcomes.
  • Must adhere to FHLC policies and procedures.
  • Regular and Reliable Attendance.
  • Other duties as assigned.


Minimum Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. This position requires an individual, who has the ability to work independently, is able to work under pressure, is analytical and has excellent problem-solving skills. Must have and maintain high ethical standards and adhere to the ethics of the profession. The ability to respect the confidentiality of patient information while performing job duties. Ability to establish and maintain effective working relationships with patients, employees and public. The ability to maintain a state of Wisconsin Driver’s License as well as the ability to pass the requirements to complete the minimum requirements to operate a company vehicle.

Education/Experience Requirements:

  • Master’s degree in Social Work, Counseling, Marriage and Family Therapy required.
  • Competency in assessment, diagnosis, prognosis, counseling, and psychotherapeutic treatment of AODA, marriage, family, and child relationship dysfunctions; methods of developing and implementing a treatment plan; current trends in the field of mental health; laws and regulations applicable to mental health; and basic math.
  • Ability to perform effective counseling and psychotherapy with individuals, families, and groups; develop and maintain cooperative relationships with patients’ families, members of other professional disciplines, social agencies, and co-workers; relate professionally with persons with serious mental disorders; identify and utilize community resources; advise and train other mental health staff; and prepare and present clear and concise oral and written reports.
  • Competency in providing cognitive and behavioral interventions to individuals, families, couples, and groups with behavioral health/substance abuse problems.
  • Excellent interpersonal skills and ability to work collaboratively with patients, substance use disorder counselors and other behavioral health clinicians.
  • Applicants must possess WI state licensure in their profession at the time of application or ability to obtain within 6 months.
  • The selected candidate for positions in this class are required to undergo FHLC’s Provider Credentialing and Privileging which includes extensive background investigation. Per Federal Grant requirements, the selected candidate must not be listed on any debarment or exclusion list.
