Job Description:
As a Certified Nurse Practitioner - CNP at Kingston Healthcare, you will provide comprehensive primary care and specialized healthcare services to our residents, collaborating with our interdisciplinary team to deliver high-quality, patient-centered care. You will play a vital role in assessing, diagnosing, and managing the health needs of our residents, promoting wellness, and ensuring the highest standards of clinical practice.
This position is on-site, based in an Ashland, Ohio community.
Responsibilities:
- Conduct thorough assessments of residents' health status, including physical examinations, medical histories, and diagnostic testing, to formulate accurate diagnoses and individualized care plans.
- Manage and treat acute and chronic medical conditions, prescribing medications, and treatments as appropriate, and monitoring residents' responses to therapy.
- Provide education and counseling to residents and their families on health maintenance, disease prevention, and self-care strategies, fostering a collaborative approach to healthcare management.
- Collaborate with the interdisciplinary team, including physicians, nurses, therapists, and other healthcare professionals, to coordinate care, address complex medical issues, and optimize resident outcomes.
- Maintain accurate and timely documentation of patient encounters, assessments, treatment plans, and progress notes in accordance with regulatory standards and best practices.
- Participate in quality improvement initiatives, evidence-based practice, and continuing education to enhance clinical knowledge and contribute to the advancement of healthcare delivery at Kingston Healthcare.
Qualifications:
- Master's or Doctoral degree in Nursing with a specialization as a Nurse Practitioner from an accredited program.
- Current national certification as a Nurse Practitioner, with specialization in adult-gerontology, family, or another related area of practice.
- Active state licensure as a Nurse Practitioner, with the ability to practice independently and prescribe medications. DEA licensed
- Previous experience in a primary care, geriatrics, or long-term care setting is preferred, but new graduates with relevant clinical rotations are also encouraged to apply.
- Strong clinical assessment and diagnostic skills, with the ability to manage a diverse range of acute and chronic medical conditions in older adult populations.
- Excellent communication, interpersonal, and teamwork abilities, with a compassionate and patient-centered approach to care.
