Education and Experience:

• Must have one of the following: Bachelor's degree in social, behavior or human services field from an accredited university; in social, behavioral, human service or health-related field, such as psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psycholgy, education or criminal justice; and have at one (1) year of experience working directly with individuals with ID
• Must be able to qualify as a Qualified Intellectual Disabilities Professional (QIDP)
• Must have valid TX driver's license and be insurable

Job Purpose:

• Performs Service Coordination to individuals such as preventing, and managing crisis situations
• Performs assessments, monitors, assessing needs, service planning and coordinating and making appropriate referrals

Job Function:

• Maintains regular contact with individuals served to monitor services and identify needs
• Service Coordinator assessments will be completed initially, annual, and/or as needed for all individuals served
• Will identify gaps in services so individual served experiences a continuum of care to the greatest extent possible
• Develops Initial and Annual Person Directed Plan and makes revision based on the needs of the individual
