Specialty: Physical Therapist

Job Description:

Provides treatment as ordered by the physician. If a therapist recommends other treatment, calls to get physician approval, and obtains signature of the recommendation in the form of initial evaluation signature or sends MD an order to sign.

Evaluates disability, injury, or disease process of each individual patient and their function.

Interprets, establishes, plans and implements proper treatment programs in accordance with the physician s referral. Re-evaluate the program every 10 visits or 30 days (whichever comes first) and makes recommendations to the patient and physician as appropriate (to include continuation of the program or discharge) for submittal.

Contact the physician directly for care as needed.

Works in collaboration with the physician and other members of the health professional team in determining the needs of the patients and in developing their total plan of care.

Documents each visit in patients chart.

Observes, records and reports in the chart and patient s reaction to the therapy program or any changes in the patient s condition, and reports to the physician as necessary.

Documents nonattendance in patient s chart.

Input and check patient charges for billing purposes.

Instructs, teaches and supervises patients, their families and other health team personnel regarding physical therapy procedures as related to the patient physical therapy programs.

Evaluates (as necessary) the home environment and makes appropriate recommendations.

Travel to different Physical Therapy clinics that are operated by Sweeny Community Hospital, to include hospital inpatients.

Participates in staff development activities and attends and participates in hospital forums/meetings. Attend and participate in monthly department meetings.

Participates in scheduled patient case conferences.

Participates in quality improvement by identification of problematic areas and active participation in problem solving process improvement.

Adheres to all rules, regulations, codes, ethics and guidelines in Federal, State and local laws concerned with the practice of Physical Therapy.

Qualification:

Bachelor s Degree in Physical Therapy from an accredited Physical Therapy Program.

Must have a current license to practice Physical Therapy from the Texas Board of Physical Therapy Examiners.

Current CPR card from American Heart Association

2 years experience in an outpatient setting preferred
