Territory:  The Physical Therapist will have a general territory within our service area where they will provide one on one in-home care to our patients.  The territory for this position would primarily be in the Northwest section of the Twin Cities Metro area (i.e. North of Highway 394 and West of Highway 65).

A few must-haves for this position:

• Current licensure as a Physical Therapist in the State of Minnesota.
• Able to travel within a geographic area serviced by office. Valid driver's license and proof of auto insurance required.
• CPR Certification

Some functions of what you will do as a Physical Therapist:

• Completes initial and ongoing assessments at appropriate time points as assigned.
• Participates in ongoing interdisciplinary assessment of the patient.
• Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides interventions and therapeutic treatment in accordance with the physician certified plan of care and physical therapy professional standards of practice.
• Counsels and educates the pateint and family to promote patient progress toward mutually established goals and in meeting healthcare related needs.
