Join our dynamic team as a Contract Registered Nurse (RN) in Chester, South Carolina! This exciting opportunity offers a 13-week contract, with the possibility of transitioning to a permanent role. As a Home Health RN, you will visit patient homes and care facilities, providing exceptional care and support. This role requires Monday through Friday availability from 8 am to 5 pm.

Responsibilities:

• Conduct home visits and care facility assessments for patients.
• Develop and implement individualized care plans based on patient needs.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain accurate documentation using HomeCare/HomeBase, Outlook, Oasis, and Teams.
• Participate in on-call rotation, typically seven to nine days per month.
• Achieve productivity targets of 30 units per week.
• Provide compassionate patient education and support to optimize health outcomes.
• Ensure compliance with all regulatory requirements and standards of practice.

Requirements:

• Valid RN license in South Carolina.
• Minimum of 6 months of experience in home health.
• Active BLS certification.
• Proficiency in HomeCare/HomeBase EMR system preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Reliable transportation and willingness to travel.
• Familiarity with Outlook, Oasis, and Teams platforms.
