Client residen in Lebanon TN. The schedule is M-Th- 7:30 am to 1:30 pm
Job Responsibilities:
Diagnoses: Parkinson's, Fall risk, Pacemaker in place, Chronic Lymphatic Leukemia, Kidney issues (catheter in place), High Cholesterol
Goals and Objectives: Each day: Assist client to toilet upon arrival, empty catheter, give bed bath, put on clean depends & assist with dressing. Shave him every other day and assist with hair grooming. Prepare and serve breakfast, Assist to take meds and vitamins, and do breathing treatment. Assist to brush teeth, clean kitchen area. Let him rest a little while and walk with him around the house for about 10-15 min, twice per shift, following closely but allowing him to slowly do as much for himself as possible. Light housework, laundry as needed. Engage with him, assist with puzzle...etc. Prepare and serve lunch, take out trash, clean kitchen area.
ADL's: Communication: Client has the ability to make his own needs met. He speaks with a very low, soft tone and slowly and his speech is sometimes slurred but he can be understood and has good cognitive reception skills, so he understands what is said to him. He has no hearing loss. He wears only reading glasses.
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
