Position Title: LEAPP Case Manager

Reports To: LEAPP Coordinator

Department: Direct Services

Program: Linkage and Engagement After Prison Program (LEAPP)

Status: Full-time, Temp position thru June 2024.

Qualifications:

  • Minimum Bachelor’s degree, Master’s preferred, in social work or related field.
  • Experience in human service delivery, case management, assessment and client advocacy.
  • Understanding of the medical and psycho-social issues related to opioid addiction and other health concerns.
  • Experience working with incarcerated and/or re-entering populations preferred. Ability to work with diverse populations.
  • Working knowledge of local resources. Ability to work with various professionals, including medical, non-medical, and para-professional staff in the case management and prison system.
  • Eligible to enter the correctional facilities as a contract employee.
  • Flexibility in work hours, including occasional evenings and weekends.

Basic Function:

  • Must have an understanding of and willingness to adhere to the philosophy and practice of harm reduction.
  • Provide case management services to individuals currently incarcerated, have an opioid addiction and those recently released from the Philadelphia County Prison system.
  • Provide consultation to other staff regarding prison issues on an as needed basis.
  • Liaison with the staff of Corizon, Defenders Association, court officers, probation and parole offices.

Specific Responsibilities:

  • Accept referrals from various sources for services to incarcerated individuals
  • Perform face-to-face needs assessment.
  • Complete quarterly service care plans with the client
  • Maintain accurate and timely activity logs.
  • Obtain updated medical and demographic data and informed consent of each client.
  • Provide case management and advocacy to individuals with an opioid addiction who are incarcerated or have recently been released from prison. This includes being available to visit the client at the prison, home, in the hospital and office visits.
  • Assist clients in negotiating, arranging and linkage to medical care, social services and other provider systems. Provide the client with information and referral, arrange necessary services. Follow up and monitor the support system on a monthly basis. Reassess client need as appropriate.
  • Work with care providers in the areas of hospital discharge planning and home care needs.
  • Provide specialized case management and advocacy by carrying a caseload of individuals whose primary presenting problem is being symptomatic of opioid use and reentry to society following incarceration.
  • Evaluate the need and assist clients to be admitted to substance abuse and/or mental health treatment.
  • Consult with other members of Action Wellness staff including direct service staff, housing specialist, prevention case managers, volunteers and others as needed.
  • Identify resources and communicate with agencies and organizations providing services to incarcerated and recently incarcerated individuals.
  • Attend weekly one on one supervisory meeting.
  • Participate in Action Wellness staff meetings and in-service trainings
  • Participate in required in service and off site trainings and staff development.

Performance Standards:

The following performance standards are specific to this position. Meeting or exceeding these standards is an expectation of employment as is compliance with all other organizational and departmental policies, procedures and expectations.

  • Required paperwork, documentation and referrals completed per the timeframes established in the Direct Services Policy and Procedure Manual.
  • Document contacts of clients on your caseload as per timeframes outlined in the Direct Services Policy and Procedure Manual.
  • Complete documentation regarding intake and information and referral inquires within 48 hours.
  • Minimum of 450 units of service per month
  • Participate in weekly individual supervisory meetings.
  • Participate in monthly team group supervision meetings.
  • Attend Direct Service and General Staff meetings.
  • Maintain awareness and knowledge of resources, benefits, and entitlements necessary to advocate and make referrals on behalf of the client.
  • Adhere to core values as outlined in Employee Handbook.
