What we're looking for:


  • Graduate of an accredited school.
  • Current LPN license in good standing.
  • Current CPR certification.
  • Reliable transportation.
  • Maintains confidentiality and HIPPA regulations.
  • Knowledge of safety and infection control practices.
What you will be doing:


  • Reporting changes for care plan updates, goals, outcomes.
  • Maintain client records showing assessments, interventions, evaluations.
  • Initiating referrals as needed to RN and careteam.
  • Basic patient care within scope of practice.
