Position Overview:

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our hospice team in San Bernardino. As a Hospice RN, you will play a crucial role in providing comprehensive nursing care to patients in the comfort of their homes or in hospice facilities. You will work collaboratively with an interdisciplinary team to ensure the highest level of comfort, dignity, and quality of life for patients during their end-of-life journey.

Responsibilities:

Conduct initial and ongoing assessments of patients' physical, emotional, and spiritual needs
Develop and implement individualized care plans in collaboration with the interdisciplinary team and the patient's physician
Provide skilled nursing care, including pain and symptom management, medication administration, wound care, and other nursing interventions
Educate patients and families on disease progression, symptom management, and end-of-life care options
Provide emotional support and guidance to patients and families throughout the hospice journey
Coordinate care with other healthcare providers and community resources as needed
Maintain accurate and timely documentation of patient care in accordance with regulatory standards and company policies
Participate in interdisciplinary team meetings, case conferences, and in-service education programs

Qualifications:

Current California Registered Nurse (RN) license
Minimum of one year of nursing experience in hospice, home health, or a related setting preferred
Excellent assessment, critical thinking, and communication skills
Compassionate and empathetic approach to patient care
Ability to work independently and as part of a collaborative team
Strong organizational skills and attention to detail
Reliable transportation and a valid driver's license
CPR certification (BLS for Healthcare Providers) required
