Responsibilities:

• Assist with personal care such as bathing, dressing, ambulation, and transfers
• Assisting client with active and passive mobility work defined by the plan of care, and documenting observed changes to the Director of Nursing
• Take and record height, weight, and vital signs as needed
• Create and promote a safe environment with adherence to established policies, procedures, and standards for quality client care
• Ensure clients rights are adhered to

Requirements:

• Active CNA certification in the state of Illinois
• High school diploma or GED
• Valid CPR Certificate
• Physical and negative TB skin test within the last 12 months, available in office
• Valid Drivers License and auto insurance with reliable vehicle
• Adhere to HIPPA and maintain client confidentiality
• Negative drug screen
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
