Job Title: FRESH Care Manager

Location: Polkton & Wadesboro, N.C (Anson County)

Hours: 40 hours/week, full-time, M-F, Exempt

GENERAL STATEMENT OF DUTIES:

Provide case management and related services centered on the child-parent relationship and the re-entry/transition process to incarcerated parents and their children both within a correctional setting and in the community.

JOB DUTIES/RESPONSIBILITIES:

• Assesses the needs of the incarcerated parent, his/her children, and the children’s caregiver(s) to develop a plan to support both the relationship and the re-entry/transition process.
• Meet with program participants individually and in groups to provide educational information to include but not limited to the following topics: communicating with your children; child development; planning for a visit or phone call; child support; supporting the child’s school life; working with child care providers; and co-parenting. This educational component will complement programming currently underway at the facility.
• Work with parents to prepare for and debrief after visits and phone calls. Work with families to provide a similar experience, either directly or in collaboration with community resources.
• Meet with children and caregivers to assess their needs, identify appropriate resources to meet those needs, and make the needed referrals (using a “warm hand off” approach).
• Participate on a team with a correctional facility case manager, probation officer, and Local Re-entry Council (LRC) staff member to ensure that parents and children are receiving the services they need as part of the re-entry/transition process.

REQUIRED EDUCATIONAL/EXPERIMENTAL QUALIFICATIONS:

• A bachelor’s degree in human services or related field. A master’s degree in social work or related field is preferred.
• One (1) year of experience working with individuals, families and children involved in a correctional setting and the community.
• Preference given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
• Ability to maintain a flexible work schedule, including travel.
• Participate in and successfully complete any required training and background check provided by the NC Department of Adult Correction (NCDAC).
• Participate in and successfully complete virtual training for Parenting Inside Out (PIO).

Salary: Salary Range for this position is $42,000-$45,000 commensurate with experience, medical/dental insurance, life insurance, disability insurance, retirement savings plan/401K, and paid time off programs.
