LSF is looking for a talented System of Care Manager of SOR and SA Services who wants to make an impact in the lives of others.

The schedule for this role is Monday - Friday during business hours.

Purpose and Impact:

The System of Care Manager of SOR and SA Services is responsible for the effective implementation of SA services and of the SOR Grant objectives. This position serves as the primary point of contact for contracted network service providers, other community agencies, stakeholders, and system partners as it relates to all substance use services including SOR, PPG, SA Block Grant and prevention.  

Essential Functions: 

  • Assists the Director of Program Operations with tasks as assigned. 
  • Interpret and administer pertinent federal and state rules, regulations, and statutes in accordance with sound clinical principles. 
  • Compile detailed reports regarding utilization management data, trends and quality improvement needs. 
  • Develop and implement system of care improvement plans that establish policies and procedures which address and correct the barriers preventing the system of care from functioning at optimal capacity. Continual and proactive review and oversight of the substance use system of care. 
  • Manage the performance of the substance use system of care. 
  • Identify and implement best practices as they relate to various substance use programs throughout the region managed by LSF Health Systems. 
  • Leverage existing resources for maximum consumer benefit by building relationships amongst community stakeholders.  
  • Monitor Network Service Provider contractual adherence and address non-adherence issues through technical assistance and potentially corrective action plans. 
  • Update contract for the programs managed annually to reflect DCF updates and best practices.   
  • Ensure oversight and execution of SOR grant objectives and compliance with mandatory project deadlines in collaboration with grant partners and community stakeholders. 
  • Ensure oversight and execution of SA Block grant, PPG, and prevention objectives and compliance with mandatory project deadlines as outlined in contract and pertinent state rules, regulations and statute. 
  • Ensure in partnership with network service providers that the State Opioid Response (SOR) Grant System Priorities as outlined in the guidance from the Department are achieved to include: 
  • Establishing Emergency Department Bridges to community-based methadone or buprenorphrine prescribes throughout the northeast region of Florida, 
  • Ensure access to Buprenorphine in all counties in the LSFHS Catchment area, 
  • Analyze and plan for sustainability, 
  • Monitor and improve retention in care, and 
  • Build peer capacity 
  • Serve as liaison between Community Providers, System Stakeholders and LSF Health Systems ensuring collaboration and communication. 
  • Work with network service providers on data collection, providing TA and training on data collection tools/assessments and instruments. 
  • Identify and implement negotiated outcome targets for SA programs as required. 
  • Coordinate, communicate, and promote data sharing among community stakeholders and agencies according to approved protocols.   
  • Ensure development of formal partnerships with legally binding agreements among agencies to provide and coordinate services as appropriate. 
  • Monitor progress in meeting performance measures. Trend data to identify gaps in care and potential opportunities for innovative programming. Using data to drive decisions, work with network service providers to identify and develop innovative programming to meet the community need. 
  • Maintain regular contact with the partner provider agencies to discuss and review requirements. When deficiencies are noted, offer TA and training to assist with compliance to grant deliverables. 
  • Work to remove barriers to access to care. Activities will include data surveillance, information sharing across regional and system partners, and development of partnerships with community stakeholders. 
  • Prepare and submit all required reports to LSFHS and the Department.  
  • Engage in the development of the agency’s plans and programs as a strategic partner. Evaluate and advise on the impact of long-range planning, introduction of new programs/strategies and regulatory action. 
  • Participate in statewide conferences and meetings as deemed appropriate for professional development.  
  • Provide leadership, direction, and management to all direct reports in order to support and enhance their ongoing development, production, and operation of their departments. 

Physical Requirements: 

Ability to travel within the LSFHS service and to LSF Health Systems headquarters in Jacksonville, FL.  Valid Florida driver’s license and current insurance. 

Education/Experience: 

Bachelor’s degree in behavioral health field required.  Master’s degree and clinical license preferred.  A minimum of two years of experience working in a behavioral health setting is required.   

 Skills: 

  • Aptitude for prioritizing responsibilities and meeting critical deadlines. 
  • Ability to work both independently and collaboratively.  
  • Demonstrated ability to communicate effectively, constructively and supportively with staff, providers and community stakeholders while also maintaining high standards of excellence.   
  • Experience in Microsoft Word, Excel, Power Point and Outlook 
  • Strong written communication skills and demonstrated ability to produce professional documents and reports 
  • Understanding of principles of quality management including data analysis and data driven decision-making, using data to measure and manage program outcomes, and using data to tell the program story
