Lutheran Social Services of Central Ohio is currently seeking a Health Educator for The Health Center at Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the Health Educator with Lutheran Social Services?

The Health Educator works with all patients and those who experience chronic illness and higher risk for health problems to improve health and wellness. The Health Educator works in a collaborative relationship with the patient and their family, caregivers, support system, other healthcare providers, and community resources using a patient-centered health approach to support the integrated care model. The Health Educator helps patients identify health and wellness goals and works toward specific objectives to improve health and deliver effective care. The Health Educator will design and develop educational programs tailored to the needs and interests of the community. The Health Educator conducts community outreach to a specified target demographic by managing LSS Health Center social media communication including Facebook and Instagram; in addition to LSS Health Center events webpage page.

  • Provide individual and group patient coaching and support.
  • Utilize health assessment activities to identify patient health education needs.
  • Collaborate with patients to develop health improvement plans centered on personalized evidence-based goals and provide support toward patients' achievement of those goals.
  • Increase patients' ability for self-management and shared decision-making.
  • Connect patients to relevant community resources, with the goal of enhancing patient health and well-being.
  • Plan, develop, and design health education and training programs, including preparation, facilitation, and follow-up.
  • Facilitate workshops, seminars, and training sessions to educate community members on various topics such as health literacy, health humility, social determinants of health, and chronic illness management.
  • Complete progress notes in an electronic health record system.
  • Contribute to the development and management of high quality, culturally appropriate digital health promotion materials.
  • Evaluate programs, performs research on relevant topics, and develops and/or revises curriculum modules and/or presentation materials as appropriate.
  • Track patient progress and program data for reporting.
  • Participate in research and other grant-funded opportunities as assigned.
  • Attend relevant training courses/webinars and meetings as assigned.
  • Provide feedback for the improvement of the Health Education Program.
  • Participate in community-based outreach events as needed.
  • Perform other duties as assigned.

Requirements for the Health Educator with Lutheran Social Services:

  • Bachelor's degree in healthcare, social work, public health, health education or related healthcare field preferred; or at least 6 months equivalent training and experience providing health education.
  • Excellent communication skills
  • Hands on experience in content management preferred
  • LSW, LPN, RN, Certified Health Education Specialist (CHES) Preferred
  • Health Education experience preferred
  • Proficiency with basic software (Word, Excel, Access, Outlook, Powerpoint)
  • Knowledge of community health services and resources
