Job Description

POSITION SUMMARY: Assists with processing and monitoring CMHC, KLOCS and DBHDID contractual obligations. Serves as a liaison for affiliate Agencies, Programs and Facilities. Coordinates services for State General Funds (SGF) to include PASRR (Pre-admission Screening and Resident Review) and Crisis.

QUALIFICATIONS: Bachelor's Degree in human services or related field. One (1) year experience working with individuals with disabilities. Preference given for prior experience with SGF services.


SKILL SETS: Good computer skills, superior oral and written communication skills, articulate, trustworthy and decisive. Other skill sets include but are not limited to highly organized, good anticipatory planning, conflict resolution and analytical skills.


PHYSICAL AND ENVIORMENTAL DEMANDS: Visual acuity necessary for updating files and completing documents. Requires frequent stooping, kneeling and crouching to file records. Hearing and talking is necessary to answer phone and talk with employees and other individuals. Work is light with walking, standing and sitting. Manual dexterity is required to perform work processing and other clerical duties.


JOB SPECIFIC FUNCTIONS:

  1. Serves as a liaison and monitoring entity for outside subcontracted agencies. Maintain pen and positive lines of communication with other providers.
  2. Comply with all applicable federal and state law requirements.
  3. Track, prepare and submit required documentation and records.
  4. Reviews and submits incident reports to DBHDID, as applicable.
  5. Provide access to service sites and participant records for the purpose of monitoring and ensuring services, health, safety, and welfare for mutually supported participants.
  6. Attend person-centered plan meetings; make positive contributions to team meetings and person-centered planning outcomes and strategies for each service site are developed.
  7. Ensure person-centered plan documents and modifications are correct and up to date.
  8. Provides quarterly monitoring of all subcontracting agencies including off-site documentation review and on-site operations.
  9. Completes extension requests following the contract protocol.
  10. Completes PASRR Level II Evaluations, assist with SGF intake process, participate in weekly crisis meetings. Assist with crisis cases and be a part of a weekly on-call rotation.
  11. Maintains a small caseload and supervises employees, as needed.
  12. Communicates status of services and potential problems to SGF Manager.
  13. Participates in agency continuing quality improvement (CQI) plan.
  14. Performs other duties as assigned
