$52000 - Case Manager

Join our dedicated team in the role of a full-time behavioral health Case Manager providing comprehensive care management services with Franklin County, Ohio. This position involves working collaboratively with nurses, practitioners and doctors to plan and oversee support for individuals suffering from mental health conditions. Work schedule is M-F, 8a-4:30p. Pay is $25/hour plus mileage reimbursement at standard Federal rates for travel related to client transport and appointments for referrals and linkages. You will be expected to document your service provision in our electronic medical records system achieving at least 1,248 patient services hours annually (104/month).
