Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.  Must maintain up to date knowledge of regulatory agency changes related to case management. Serves as a care coordinator to monitor and coordinate a comprehensive program of admissions reviews, continued stays and discharge plans of the patient.  Processes appropriate data to monitor and guide acute care hospital coverage for patients.  Conducts medical record audits and evaluations.  Prepares statistical analysis in accordance with departmental, organizational, and regulatory agencies requirements.  Initiates and directs care management of patients from preadmission through discharge for appropriate movement through the continuum of care while providing quality care advice to meet specific patient needs.  Monitors cases, develops and implements plans for appropriate documentation and length of stay compliance.

Minimum Qualifications:

Minimum Education
Graduate of accredited School of Nursing. RN required.
  
Required Skills

Ability to communicate effectively. Knowledge of Basic Computer Skills. Ability to work under high stress situations and maintain professional attitude. Ability to accept responsibility. Ability to follow policies and procedures. Demonstrates discretion in handling materials of confidential nature.

  
Required Skills
Certifications:

Current AHA BLS is required.
Prefer Certification as Case Manager.

Requires full range of motion including manual finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 25 pounds. Requires corrected vision and hearing to normal range. Requires working under stressful conditions and/or irregular hours. Potential for exposure to communicable diseases and/or body fluids.
  
Required Skills
Licenses: Current licensure by Virginia State Board of Nursing.
  
Minimum Work Experience
At least one year of clinical direct patient care nursing experience in an acute care hospital required. Previous Nursing/Case Management/Utilization Review/Discharge Planning experience preferred.
