About who needs care: Recently discharged from rehab. Broken foot and wrist. Limited mobility. Can make it to the bathroom and bedroom on her own with a walker; with difficulty.
Struggling with motivation to get up and move.
Need help with bathing, hygiene and feeding.
About the care needs: Friendly, conversational but not overly talkative. Patient is quiet.
Ability to encourage patient to move and exercise.
Services needed include: transportation, feeding, and bathing / dressing.
