Salary: $21 - $26 per hour DOE

Job Summary: The Housing Navigator is responsible for providing support and assistance to members and their families experiencing homelessness or at risk of losing their housing.


Position Responsibilities:

  • Researches, establishes, and maintains a list of available housing opportunities in the region.
  • Develops relationships with and becomes knowledgeable about local community resources including transitional housing, shelters, and permanent housing.
  • Develops individualized housing plan and communicates the plan and housing opportunities to the member to ensure effective service and that any placements are consistent with the members overall care plan.
  • Assist members with their housing search, including applying for apartments, negotiating with potential landlords, filling out rental applications,
  • Assist members in understanding tenant rights and responsibilities.
  • Assist members with talking to their landlord by being a mediator if there is a disagreement or concern.
  • Conducts follow-up meetings with members after housing placement to assist with any issues that may have arisen.
  • Maintains accurate documentation of service objectives, outcomes, and other member related information in the appropriate data systems.
  • Participates in collaborative team meetings to provide input, support, and brainstorm solutions to common areas of concern and develop best practices.
  • Understands and abides by all departmental and companywide policies and procedures.
  • Complies with all safety and injury prevention policies and regulations.
  • Performs other duties as assigned or required.
  • Utilize the HMIS: Homeless Management Information System to add members into housing portal.
  • Assist members in completing CES assessments. Whether completing it with them on HMIS or directing them to a CES Access point where they can get the assistance to complete the CES assessment.
  • Assist members with in-person assessments or housing tenancy.
  • And other duties assigned


Position Requirements:

  • Education: 1-2 years’ experience in healthcare, social work, public health, physiology, or other related field OR Associates Degree in healthcare
  • Experience: 1-2 years relevant experience – such as social work, case management, or working with unhoused and unsheltered individuals/families and veterans’ programs.
    • Experience working with HMIS preferred.
    • Experience in a social service setting with working knowledge of case management systems and planning techniques preferred.
  •  Language: Fluent in English (written and verbal). Preferred bilingual to address the Spanish-speaking population
  • License/certification: Current and valid Driver’s License and proof of auto insurance.
  • Skills required:
    • Competent with computers, email, virtual platforms, Excel and other Microsoft Office based programs.
    • Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear, effective manner.
    • Ability to identify problems and use logic and related information to develop and implement solutions.
    • Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures.
