Salary: Commensurate with experience

                                                                                                 TARGETED CASE MANAGER


POSITION SUMMARY:

The Targeted Case Manager will carry a caseload that will provide case management, support, advocacy, community resources, ongoing referral and assessment for services provided and/or needed for each client. Provide quality case management to individuals who are recipients of mental health services and their families as outlined by the state and in accordance with state and federal laws, regulations and company policy and procedures. This position reports to the Clinical Director in accordance with company and regulatory policy.


GENERAL RESPONSIBILITIES:


  1. Must meet at a minimum of 4 contacts monthly, at least 2 must be face to face with client
  2. Maintain a full-time caseload of 25 cases
  3. Review treatment goals and objectives and assist clinician with developing Crisis Prevention Plan, and strength-based needs assessments with client and family and document as required by state and office regulations
  4. Complete contact notes within 48 hours
  5. Ensure initial legal documentation is obtained prior to placement or service delivery
  6. Must review treatment goals and objectives quarterly and make necessary changes after a team meeting
  7. Obtain knowledge on local resources in the client’s county of residence
  8. Understand the clients’ diagnosis and the challenges it presents to their family and provide information, support and/or intervention to the client and their family
  9. Meet with other service providers to plan and coordinate treatment to ensure the integration of the treatment across the client’s home, school, and community and to achieve the desired outcomes and goals identified in the treatment plan
  10. Participate in continuous improvement of programs and services for the organizations
  11. Demonstrate ethical problem and conflict resolution skills
  12. Present and always perform in a professional manner and dress
  13. Facilitate interdisciplinary treatment team meetings when needed.
  14. Provide direct care and treatment for individuals, families, and groups which will include linking the identified resources, monitoring services received, advocating for client needs, discharge planning and follow-up. Will also provide support to the clients' family members if applicable
  15. Document, review, and update treatment plans as necessary.
  16. Submit all required documentation and maintain client charts as required by state and federal regulations and company policy
  17. Must understand how to use computers such as a laptop/chrome book
  18. Must have computer knowledge and skills in using web-based applications such as Gmail, google documents, spreadsheets, google drive, outlook, and google calendars, and other web-based applications.
  19. Must be able to understand company policies and educate clients in those policies
  20. Must be able to work in a team environment and independently
  21. Must be able to follow through with all tasks to completion
  22. Must have timely and effective communication skills
  23. Work hours vary based on the client/family's schedule
  24. Traveling required, must have own reliable transportation
  25. Completes all required trainings
  26. Other duties as assigned


BACKGROUND/ EDUCATION:


  1. Bachelor of arts or science degree in a human services or behavioral science field with One Year post-educational work experience working with adults in a human service setting.
  2. Successfully complete a company approved targeted case management training within six (6) weeks of employment as a Targeted Case Manager.
  3. Ability to work in a fast paced, dynamic environment.
  4. Valid Kentucky driver’s license.


QUALIFICATIONS REQUIREMENTS:


Bachelor’s Degree in psychology, social work, or mental health; preference for work experience with the targeted population assigned within the human service setting. Minimum one (1) year of experience post degree working with persons in a human service setting. Adequate communication skills, process verbal and written instructions, dependable, interact effectively with the public and staff, ability to adapt to frequent change, delays or unexpected events, keen sense of commitment.


SKILLS REQUIRED:


  1. Excellent organization and time management skills.
  2. Effective communication skills, both written and verbal.
  3. Ability to maintain positive interpersonal relationships.
  4. Must have the ability to work independently and as a team and perform consistently in a stressful and busy environment.
  5. Work collaboratively with the client and treatment team to develop a comprehensive care plan that addresses priority needs of the patient, desired outcomes, an agreed upon plan of action, and any needs for reassessment.

Physical Demands:

Must be able to sit, stand, bend, reach overhead and lift to 10 pounds; travel outside of the office and/or attend meetings both during and after normal work hours, both at and away from the work site. Exerting up to 10 pounds of force occasionally or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. May involve walking or standing for brief periods of time. Requires about 6 hours of sitting and no more than 2 hours of standing or walking in an 8-hour workday. No significant stooping is required.

 

Environmental Conditions:

The employee will work indoors in a residential setting with central heating and air conditioning. The house is open and well ventilated, with central heating and air conditioning. The person may work outside in heat/cold, wet/humid, and dry/arid conditions during times of community activities for clients.


Mental Demands:

There are several daily deadlines associated with this position, which may cause significant levels of stress to some individuals. The position must deal with clients and community representatives daily on a variety of issues.


WORK EXPECTATIONS:


  1. Minimum of 40 hours per week
  2. Must be able to work well with other staff and clients
  3. Must be able to travel independently locally
