The Tampa Bay Academy of Hope is steadfast in its mission to facilitate a seamless transition for veterans returning from incarceration back into the community. Through our 2nd Chance Heroes Program, we stand as a robust pillar of support and guidance for veterans, aiding their reintegration journey and ensuring that they forge a path toward stability and purposeful living.


Position Overview:

As a Case Manager, your role anchors on providing vital, ongoing support to veterans on their path from incarceration to community reentry. Through relationship-building, empathetic support, goal creation, and comprehensive resource provision, your endeavors will lay down the stepping stones towards meaningful and sustainable reintegration for the veterans we proudly serve.


Key Responsibilities:

  • Participant Engagement: Develop strong, supportive relationships with program participants, ensuring their needs, challenges, and aspirations are acknowledged and addressed.
  • In-Prison Involvement: Willingly and effectively navigate correctional facility environments to build initial rapport and plan with veterans prior to their release.
  • Goal Development: Collaborate with veterans to establish and pursue achievable, individualized goals, paving the way for their successful reentry into the community.
  • Resource Provision: Identify, connect, and follow up on referrals to crucial resources, services, and community supports that will aid participants in their journey.
  • Travel and Outreach: Frequent travel across designated counties to engage with participants, community resources, and relevant entities, ensuring comprehensive on-ground support.
  • Document Management: Meticulously record and manage participant data, maintaining confidentiality and accuracy in tracking their journey and outcomes.
  • Team Collaboration: Work in synergy with internal teams and external agencies, coordinating a holistic approach to participant support and resource provision.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Criminal Justice, or related field.
  • Demonstrated experience in case management, particularly within contexts related to veterans, returning citizens, or similar populations.
  • Willingness and capability to work within correctional facilities and provide direct support to incarcerated individuals.
  • Strong interpersonal skills, with the ability to build and sustain relationships.
  • Valid driver’s license and ability to travel across the specified counties.

Preferred Qualities:

  • Knowledge and understanding of the specific challenges faced by veterans, particularly those navigating reintegration post-incarceration.
  • An innate ability to approach situations with empathy, practicality, and a solutions-oriented mindset.
