Lighthouse Behavioral Health Solutions (LBHS) provides individual, outpatient, intensive outpatient, partial hospitalization, residential, psychiatric services, and medication assisted treatment for mental health and substance abuse treatment needs. Position: Care Coordinator 

  

Job Summary: 

This position provides case management services to adults with identified mental health or substance use diagnosis. This position reports to a Housing Supervisor. 

 

Duties and Responsibilities: 

Duties include, but are not limited to: 

  • Provide ongoing case management services to residents, including regular check-ins, goal setting, and progress monitoring. 
  • Serve as a liaison between the recovery housing program and treatment providers, facilitating communication and information sharing regarding case management services. 
  • Coordinate and schedule appointments with treatment providers, ensuring residents have access to necessary services and follow-up care. 
  • Assist residents in accessing community resources and support services, such as employment assistance, educational programs, and legal services. 
  • Advocate for residents' needs and rights, ensuring they receive appropriate and quality care. 
  • Conduct regular meetings with residents to address common challenges, provide education, and promote community-building. 
  • Collaborate with the recovery housing program staff to ensure a safe and supportive living environment for residents. 
  • Maintain accurate and up-to-date documentation of resident interactions, progress, and treatment plans. 
  • Participate in team meetings and treatment team meetings to discuss resident progress and develop strategies for improved care. 
  • Provide crisis intervention and support to residents in times of relapse or other emergencies. 
  • Conduct urine screens and medication counts at the request of clinical staff. 
  • Coordinate discharge planning and support residents in transitioning to independent living or other appropriate levels of care. 
  • Stay updated on current trends, research, and best practices in the field of addiction recovery and mental health. 
  • Collaborate with external agencies, community organizations, and treatment providers to enhance the resources available to residents. 
  • Attend training sessions, workshops, and conferences to expand knowledge and skills relevant to the role. 
  • Uphold ethical standards and confidentiality guidelines in all interactions and documentation. 
  • Contribute to program evaluation and quality improvement efforts, providing feedback and suggestions for enhancement. 
  • Foster a culture of respect, empathy, and support within the recovery housing program. 

 

Required Experience/Abilities: 

  • Possesses superb written and spoken communication skills 
  • Excellent interpersonal skills with clients, colleagues, and community resources 
  • Excellent time management skills; organized and able to prioritize 
  • Comfortable in a fast-paced environment with multiple clients 
  • Able to organize and manage large amounts of files, schedules, dates, and information 
  • Calm and cool-headed with a positive attitude 
  • Self-directed and self-motivated 
  • Comfortable with building personal relationships and dedicated to helping others 
  • Valid State of Ohio Driver's License and car insurance
  • Must pass BCI check, all Corporate Compliance checks, and employment drug screen

 

Desired Certifications and Experience: 

Required: 

  • CDCA or LCDCII 
  • State of Ohio Driver's License 

Preferred: 

  • LSW 
  • Bachelor’s Degree in a behavioral health field (Social Work, Psychology, Counseling, etc.) 

 

Schedule: 

Tuesday - Saturday or Sunday - Thursday 12pm - 8:30pm
