Care Manager RN, Newark, DE
- Coordinate Care: Manage and coordinate comprehensive healthcare services for patients, ensuring continuity and efficiency throughout their treatment process.
- Assessment: Conduct thorough assessments of patients' medical, emotional, and social needs to develop personalized care plans tailored to their individual requirements.
- Patient Advocacy: Serve as an advocate for patients, liaising between healthcare providers, insurance companies, and other stakeholders to ensure patients receive appropriate and timely care.
- Resource Management: Optimize the use of healthcare resources by facilitating access to community resources, coordinating referrals, and advocating for cost-effective treatment options.
