Job Description Summary

The Nurse will be under the general guidance of the Medical Director of the Breast Care Center and Multispecialty Clinic and will administratively report to the Practice Manager, in accordance with departmental and hospital policies. The Nurse provides individualized care coordination to patients, their families, and caregivers throughout the continuum of their care. The Nurse functions as the primary point of contact for patients and supports patients throughout all levels of their care, including diagnostic evaluation, treatment, and post-treatment. They are responsible for coordinating care, monitoring, and educating patients as they move through our healthcare system. The Nurse helps to facilitate timely access to quality medical and psychosocial care from screening, treatment, and cancer survivorship. They are a member of the multidisciplinary care team that requires effective communication and collaboration with colleagues in the Cancer Center, as well as other collaborating institutions. The role requires critical thinking, clinical expertise and assessment skills, collaboration, and excellent communication skills.

Job Description

ESSENTIAL FUNCTIONS
1 - Delivers expert, evidence-based nursing care to patients and families
  A - Implements nursing process:  Assesses patient and family needs; Analyzes assessment data to develop and support clinically accurate diagnoses; Plans care by establishing goals with measurable outcomes; Implements the plan of care; and evaluates the patient’s response to interventions.
B - Documents care according to Institute and regulatory agencies’ requirements.
C - Evaluates educational and psychological needs of patient and family and incorporates identified needs into the plan of care.
E- Educates patient and family regarding various aspects and treatment modalities of disease, which may include chemotherapy and will include radiation therapy. 
 F - Recognizes and responds to emergency situations appropriately  
G- Identifies appropriate community resources to promote continuity of care and makes appropriate referrals.
H - Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role
I – Triage pt phone calls appropriate based on needs of call
J - Ability to function in a busy work setting with patients with complex needs.


2 - Establishes rapport with patients and families from a variety of cultural, ethnic, and religious backgrounds.
a - Interacts with patients and families in a caring and compassionate manner respecting the need for privacy and confidentiality.
b - Develops approaches to problem solving which meet the individual needs of the patients and families.

   b - Participates in unit, program-based, and divisional committees, councils, and initiatives.
   c - Demonstrates flexibility in setting and changing priorities.  Accepts additional responsibilities as needed.
   d - Participates in QA/QI projects for the unit and the Division of Nursing and Patient Care Services.
e - Supports research activities within the unit, Division of Nursing and Patient Care Services, and the Institute.

5 - Works collaboratively and functions as an effective member of the health care team.
a - Demonstrates the ability to work collaboratively with peers and co-workers. Effectively addresses and manages conflicts.
b - Demonstrates effectiveness as a contributing member to the multidisciplinary health care team.
c - Delegates appropriate and supervises assistive personnel in their provision of patient care.
6 - Seeks opportunities to foster professional growth.
a - Identifies areas for professional growth.  Formulates professional goals, objectives, and methods for accomplishing these.

JOB REQUIREMENTS
1.    Minimum Education -Graduate of an accredited School of Nursing as validated through the Massachusetts Board of Registration in Nursing
a.    Certification in Oncology Nursing preferred – OCN, AOCN, CPON
2.    Minimum Work Experience
a.    RN for at least 3 years in outpatient setting or med surg/oncology/case management background.
3.    Must be available to work at least 2-3 shifts every month.
