Description:

Medical Records personnel are responsible and accountable for the maintenance of patient health records. This includes the structure and consistency of related patient databases, the accuracy and integrity of acquired images and the successful integration of utilized hardware and software.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Ensures confidentiality of all medical records information and completes forms for release of information.
  2. Responsible for the conversion of existing medical records into electronic format as needed.
  3. Maintain the integrity of multi-dimensional databases.
  4. Perform patient data research and extractions.
  5. Answer patient phone calls to the Medical Records phone lines, efficiently, professionally and handle/forward as appropriate.
  6. Paging or getting other providers on the line for Providers, Managers and Staff as requested.
  7. Provides charts/documents requested for use in legal actions, following patient consent/confidentiality protocols.
  8. Protects patients' health information for confidentiality, authorized access for treatment and data security.
  9. Scanning and assigning to the correct queue within the EMR clinical inbox homepage.
  10. Knowledge of the use and maintenance of office equipment (telephone, printers, fax machine, computers, scanners, etc.).
  11. Reviewing and routing electronic documents requiring clinical action to appropriate parties.
  12. Destructing records as retention policy dictates.
  13. Deactivating and managing deceased patient records.
  14. Meets quality standards and productivity requirements and project deadlines.
  15. Perform any other related duties as required or assigned.
Requirements:

EDUCATION AND EXPERIENCE

High school diploma or equivalent. Prior medical records experience with a clinic, skilled nursing or healthcare facility required. Ability to navigate an EMR system. Knowledge of medical terminology require. Prior work experience in release of information is a plus. Medical Assistant or LPN highly preferred for this role.
