Our Home Wellness program is hiring a PRN RN. NO weekends, NO evenings, NO holidays! Make your own schedule. This part-time role will be 10-15 hours/ month.

Job Summary:

The Home Wellness team Nurse serves clients, as part of a multi-disciplinary team. Complete health assessments and follow up that includes, but is not limited to, discussion of medical issues with client/primary care physician, client education health management. Advocate for and with clients through necessary disciplines, coordinating care with other medical service providers as client progresses in treatment. Clinical supervision of field staff.

Duties & Responsibilities:

· Conduct client education about medical issues.

· Complete health histories and health assessments.

· Provide documentation in the Health Record.

· Coordinate and conduct prevention activities.

· Develop & implement individual care plans.

· Crisis intervention, as needed.

· Assist clients with coordinating medical appointments with other health professionals as needed.

· Oversee Personal Care Aide and Homemaker services on regular basis.

· Provide training and education as required to Personal and Home Care Aides.

· Communicate with Case Managers and physicians as needed.

· Complete APS reports as needed.

Education and/or Work-Related Experience Requirements:

· Registered Nurse with Valid Ohio license required.

· Current first aid and CPR certification.

· De-escalation and crisis intervention training completed.

· Experience in public health services.

· Driving Company Vehicles: No

· Driving Personal Vehicles: Yes

· Computer Competency: Yes

· Valid Ohio driver's license and insurability by the company. (Requires Annual Assessment)

· Completion of all required trainings.

· Clean Ohio MVR.

· Ability to pass periodic background. investigation as required by funding source
