Position Summary:

The PACT Support position is a supportive role to the clinical teams providing community based services to individuals with mental health and substance use diagnoses. The PACT Support provider engages in one-to-one meetings with PACT clients for the purpose of planning and engaging in various tasks, supporting clients in getting to appointments, and engaging in social and recreational activities. The PACT Support provider typically does not attend daily report meetings, but can be invited by the teams to various clinical meetings and are responsible for keeping close communication with the client's primary and/or clinical director to ensure collaboration and alignment with the goals and philosophy of the team and client. Most PACT support services are provided during evenings and weekends, but alternative schedules will be reviewed on an as needed basis. This is a 5-10 hour per week position.


Essential Functions and Responsibilities:

  • Work in coordination with the Clinical Director and PACT Support Manager to provide one-on-one support to Ellenhorn clients. This would include (but is not limited to) support with accessing the community, participating in events and activities, cooking and meal preparation, tasks of daily living (grocery shopping, transportation, organization of apartment, etc), medication observations, and engaging in social opportunities.
  • Participate in weekly PACT support group supervision and other meetings with team as needed
  • Maintain accurate written documentation of client interactions daily.
  • Adhere to all HIPPA and all federal client privacy regulations.

Nonessential Functions:

  • Occasional interstate travel

Physical Demands and Work Environment:

  • Physical Demands: While performing the duties of this job, the employee is often required to walk, sit, drive a vehicle, use hands to type, handle or feel objects, reach with hands and arms, bend, walk up and downstairs, talk, hear, and see
  • Work Environment: Most work is performed outside of the office setting
Requirements:
  • College degree in Human Services field, or in another area, have a high school diploma, or have at least two years of paid or volunteer work experiences with adults with severe and persistent mental illness.
  • Must have a valid driver's license, dependable automobile, and a clean drivers record
  • Availability to work evenings and weekends
  • Have skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for the clients' rights and personal preferences in treatment
  • Strong interpersonal and communication skills
