THE POSITION
Washington County Behavioral Health Division within the Department of Health and Human Services is recruiting for a Forensic Exceptional Needs Care Coordinator. This position will be providing care coordination to individuals under the who are determined as meeting criteria as per contract with the Oregon Health Authority. This position will also be conducting Civil Commitment investigations and Trial Visit monitoring.

About the Opportunity:
This position will primarily serve adults with complex behavioral health needs under the Aid and Assist program who are meeting Choice Program eligibility with the primary goal of collaboration with the individual, system providers/partners, and other organizations providing supports. This role will also be conducting Civil Commitment investigations and will be a bridge between the Choice and Civil Commitment Programs.

This position is eligible for a hybrid work schedule including working 2-3 days/week remotely.

  • Experience and commitment working with adults with complex behavioral health needs.
  • Experience working with clients who are accessing higher levels of care in the community.
  • In-depth understanding of Civil Commitment law.
  • Experience interfacing with hospital psychiatric providers.
  • Knowledge of providing behavioral health assessment and risk assessment.
  • An understanding of the needs of the Black, Indigenous, People of Color (BIPOC) community in Washington County.
  • Strong organizational skills and the ability to prioritize and manage a variety of tasks in a fast-paced environment.
  • Ability to complete documentation accurately in a timely manner.
  • Strong collaboration skills and the ability to build and maintain relationships with clients and system partners.

ESSENTIAL JOB DUTIES

Forensic Exceptional Needs Care Coordinator is a working title. This position is classified as a Senior Mental Health Services Coordinator. Classification descriptions are typically written broadly. To review this classification, please use this link: .


Responsibilities include, but are not limited to:

  1. Meeting regularly with individuals open to Choice Program to coordinate care and develop plans to determine placement needs and support that may aid in the success of placement at the lowest level of care possible.
  2. Serving as an informational resource for behavioral health providers and other community partners for assigned clients; specifically, regarding the history of needs in care and approaches that have proven successful.
  3. Serving as an informational resource for involved family members and assigned guardians.
  4. Participating in staff meetings in person or by phone for individuals assigned.
  5. Developing knowledge of the service continuum available to specific clients open to Choice Program in Washington County.
  6. Identifying specific services and supports that would be of a benefit to assigned clients
  7. Providing Pre-Commitment services to those diagnosed with serious and persistent mental illness (SPMI) for the purpose of determining the need for involuntary hospital care.
  8. Assessing individuals and determining if specific legal criteria are met for civil commitment. Based on the timelines set in Oregon Statutes, preparing thorough reports that are professional and support the determination made regarding commitment recommendations.
  9. Providing Trial Visit monitoring for individuals who have been civilly committed by the court and are completing commitment in the community.
  10. Traveling to various settings to meet with individuals and staff, including acute care facilities.
  11. Maintaining positive, professional relationships with agency providers, system partners and hospitals serving this client population.
  12. Providing coverage for co-workers during their absence to assure services are not disrupted due to such absence.
  13. Documenting all services provided as described above and ensuring that documentation is current in the event someone would need to cover for an absence.
MINIMUM QUALIFICATIONS

Education and Experience:

  • A master's degree in a clinical field related to the diagnosis, etiology and treatment of mental health disorders; AND three (3) years of increasingly responsible mental health experience in assisting individuals with severe and persistent mental illness in accessing needed services and support; AND
  • Must be certified by the Mental Health and Addictions Certification Board of Oregon as a Qualified Mental Health Professional or hold an approved clinical license and meet the criteria of a Qualified Mental Health Professional as defined in the Oregon Administrative Rules.

Additional Requirement:
  • This position requires to receive the state certification as a Civil Commitment Investigator/Examiner within 60 days upon hired.
