Family Specialist, Flemington, NJ, (21757) Description Salary - $40,950 per year Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Hunterdon and Mercer County. Responsible for establishing a trusting relationship with families and working with them to identify their service goals. Uses a holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services. Rotating 24/7 coverage required. Case management and case documentation. Case conference with CP&P and other providers. Remote work possible after 3-month probationary period. Requirements Bachelor’s Degree in related field required. Requires a flexible schedule including availability on evenings. Driver’s license needed with an acceptable driving history.
