Telephonic Nurse - Practical nurse

The Care Manager, Telephonic Nurse 1, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 1 work assignments are often straightforward and of moderate complexity. The Care Manager, Telephonic Nurse 1 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Required Qualifications:

• 2-3 years of relevant experience
• Practical Nurse (LPN) in Puerto Rico with no disciplinary action.
• Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

Preferred Qualifications:

• Medication experience.
• Prior experience with Medicare recipients.
• Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems.
• Experience working with geriatric population.
• Experience with health promotion, coaching and wellness.
• Bilingual is a plus.
