Job Description
· Reports directly to Day Habilitation coordinator.
· Assure all procedures are implemented that program meet federal and state regulations policies and procedures.
· Monitor and Record individual progress to ensure that goals and objectives are met.
· Completes daily individuals progress notes basis according to standard.
· Writes monthly summaries for each individuals served.
· Inspect vehicle prior to use to ensure that they are in safe operating condition.
· Develop individual activities according to Staff Action Plan.
· Responds to all statements of deficiencies issued by the state.
· make sure your area of work is clean and condition to work with participants
· Provide day hab services face to face to assigned individual.
· Responsible to maintains annual trainings update
· Report and complete notables as necessary.
· All other duties as assigned by the program supervisor.

- Skills and Abilities:
- High School diploma/GED
- One year experience working with diverse clients in a human services setting preferred
- Preferably bilingual (English/Spanish)
- Valid NYS Driver’s License/Reliable transportation
- A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.
- Physical Demands:
- The position does require occasional standing, squatting, lifting of up to approximately 10 lbs. and frequent sitting.
