Summary

The Patient Referral Coordinator works under general supervision and is responsible for the coordination of new patient referrals, to include submission, tracking, patient scheduling, and closing the loop on all referrals as well as the coordination of treatment for hospital and/or ancillary services. Entails communicating with clinical staff, providers as well as payors and patients. The New Patient Referral Coordinator ensures an efficient referral process which provides a seamless consistent process of referral for the patient across the Cardiovascular Service Line.

 

Position Reports To: Clinic Administrative Supervisor


Essential Functions

  1. Manage referrals in a complete and timely manner with special attention to urgent conditions/requests.
  2. Coordinate supporting documentation to facilitate and coordinate patient care involving agencies outside of Confluence Health.
  3. Uses computer system (Epic) to match physician availability with patient's preferences in terms of date and time.
  4. Conduct outbound calls to patients that have not called to schedule their appointments. As well as contacting provider's offices for general inquiries regarding referrals/orders received.
  5. Ability to interact with staff (at all levels) in a fast-paced environment, sometimes under pressure, remaining flexible, proactive, resourceful and efficient, with a high level of professionalism and confidentiality.
  6. Uses time efficiently, able to deal with frequent change or unexpected events; must be able to handle multiple tasks with interruption; work independently.
  7. Manage records confidentially in accordance with medical center policies/procedures and RCW’s.
  8. Ability to maintain composure & balance multiple priorities in a fast-paced environment.
  9. Critical thinking and problem-solving skills to identify and resolve issues in a timely manner.
  10. Demonstrated ability to communicate effectively and tactfully.
  11. Excellent organizational and follow-through skills, attention to detail, able to prioritize and balance multiple assignments.
  12. Participates in department and/or corporate process improvement activities serving as a resource on patient flow.
  13. Knowledge of customer service principles and practices.
  14. Follows appropriate channels of communication.
  15. Other duties as assigned.

Position Requirements

Qualifications

Required:

  • High school diploma or equivalent.
  • Ability to prioritize assignments and effective time management skills.
  • Must be detail oriented, flexible and committed to providing excellent customer service.
  • Ability to work independently and as part of a team with environmental directions.
  • Demonstrates skills in planning, organizing, and managing multiple functions and complex processes.

Desired:

  • At least 1 year of experience in a medical office or hospital setting.
  • Medical terminology; basic knowledge of frequently ordered diagnostic tests.
  • Strong communication skills.

Working Conditions:

  • Work is performed in an office environment consisting of sitting at a desk for long periods of time, talking on the phone or working on computers or paperwork. This position requires a combination of working independently and working closely with others. Hours are rarely irregular; very minimal overtime may be required. Requires working under stressful situations. General supervision is provided by the Practice Manager, with additional instructions, guidance and directions provided by the providers and clinical staff.
