HOST Case Manager


LOCATION: HOST Program | 1143 Liberty St NE

YOUR ROLE:

Through positive support, motivational techniques, and encouragement you will provide action-based case management to homeless and street-oriented transition-age youth ages 18-24 years old.  You will work closely with HOST clients by providing appropriate referrals and access to healthcare and by developing strength-based case plans to promote growth toward self-sufficiency.  Help facilitate activities and skills trainings with clients to promote the skill acquisition necessary to obtain and maintain long-term housing, financial stability, and independent living. You will work collaboratively with all agency departments and other community partners to improve outcomes for youth.

 

The Case Manager position provides information, including information about, and assistance with the VOCA program and victim’s compensation program, assistance, and referral services in effort to coordinate care and connect each individual homeless crime victim with appropriate services.

 

SPECIFIC DUTIES:

  1. Responsible for the development, implementation, and monitoring of individual support plans and outcomes. This includes modifying plans and skills training curriculum accordingly and recording performance and outcomes according to the TLP manual.
  2. Complete documentation requirements of the program and position, such as completing screening tools, assessments, process notes, outcome tracking forms, reporting and documentation of care plan actions in a timely manner within the Electronic Healthcare Record (EHR) System.
  3. Supports the Resource Center function and services, including drop-in support for consumers, and covering the floor as needed to help maintain a safe environment.
  4. Supports the intake and orientation process of the TLP by working with youth to acclimate to the program and complete identified tasks. Will work closely with program staff to assess areas where TLP clients need assistance and to help create plans that support clients.
  5. Knowledgeable of community resources and refers appropriately. Develops close partnerships with other community providers to coordinate care and improve outcomes.


QUALIFICATIONS:

  • Bachelor’s degree in psychology, social services, or related field required. An Associate’s degree in social services or related field with comparable experience and training working with youth may be considered as well.
  • Minimum 1-year experience working with youth (preferably at-risk youth).
  • Demonstrated ability to work effectively as a member of an integrated team.
  • Strong verbal and written communication skills.
  • Strong organizational abilities.
  • Demonstrated ability to provide unconditional positive regard and empathy toward other people.
  • Valid Oregon Driver's License and proof of automobile insurance.
